To make an appointment please call the office on 08 9212 4269. Please ensure you obtain a referral from your GP or specialist before making an appointment. You are required to have a valid referral before seeing Richard. We have a private clinic every Monday afternoon and all day Fridays. For urgent cases, we will ensure you are seen as soon as possible.

Patient’s first visit
On your first visit to see us please bring the referral from your GP or other specialists. You should also bring the following items if applicable:
- Medicare details
- DVA or pension card
- Workers compensation, claim number, and case manager details
- Private health fund details
- X-rays, Ultrasounds, MRI, and CT reports
- Blood results or any other relevant information
Patient Information Form
Most patient details can be supplied via our Patient Information Form. This form can be downloaded, printed, and completed prior to arriving or we will have forms that can be filled out when you arrive – please allow 10 – 15 minutes to complete the form.
Payment Policy
Our staff will be happy to provide you with a cost estimate for your surgery. Richard is a KNOWN GAP provider with most private health funds. Please be advised that the anaesthetists we work with may charge a gap fee. The anaesthetist will talk to you personally to discuss the fees.
